Booking a first visit, start to finish.

  1. Book the appointment

    Call (949) 365-0403 or send a request through the website. Tell the office what hurts and roughly when you are free. Download the intake form below and fill it in before you arrive, and the first visit starts ten minutes sooner.

  2. Arrive ten minutes early

    Bring ID, your insurance card if you have one, and any imaging or reports from other providers. Wear something you can move in. Free parking is directly outside; the office is Suite 150, on the ground floor.

  3. The examination

    You choose the path. Relief quiets the symptom; correction addresses the cause and takes longer. Either is a legitimate choice — you just make it knowing which one you picked.

What if you can't help me?

He will say so and refer you to the proper specialist. That answer comes at the beginning, not after ten visits.
